Register as an Athletic Trainer

By submitting this registration, you confirm and agree to all of the following:

1. All credentials, license numbers, BOC certification numbers, and information provided are accurate, complete, and currently valid at the time of submission. You understand that CATN does not independently verify credential information and that you are solely responsible for the accuracy of all submitted information.
2. You are an independent professional and not an employee, agent, contractor, or representative of CATN. No employment, agency, partnership, joint venture, or other business relationship exists between you and CATN by virtue of your listing.
3. You carry appropriate professional liability insurance for the services you provide, and you are solely responsible for maintaining adequate coverage.
4. You understand that your listing requires annual reverification. You will be required to resubmit your credentials once per year from your approval date to maintain an active listing.
5. If you practice in a state that requires athletic trainer licensure, you confirm that you hold a current, valid LAT (Licensed Athletic Trainer) credential in that state. You are solely responsible for understanding and complying with all licensure, scope of practice, and supervision requirements in every state in which you practice. CATN makes no representation about your compliance with any state's requirements.
6. You accept sole and complete responsibility for any and all services you provide to clients who contact you through this directory. CATN has no role in, and expressly disclaims all liability for, any service relationship formed between you and any client.
7. You agree to indemnify, defend, and hold harmless CATN, its owners, operators, and representatives from any and all claims, damages, losses, costs, or expenses (including reasonable legal fees) arising from your services, your listing, or any misrepresentation of your credentials or qualifications.
8. The Concierge Athletic Trainer Network is a listing directory only. It does not employ, supervise, endorse, recommend, or accept liability for the professional conduct or services of any listed practitioner.
9. Misrepresentation of credentials, license status, scope of practice, or any other material information is grounds for immediate and permanent removal from the directory.
10. You authorize CATN to display the information you provide (including name, credentials, contact information, location, specialty, bio, website, social media links, and travel availability) publicly in the directory upon approval.
