Annual reverification acknowledgment

  1. All credentials, license numbers, and information are accurate and currently valid.
  2. Your BOC certification (ATC) is current and in good standing.
  3. If your state requires licensure, your LAT credential is current and valid.
  4. You continue to accept sole responsibility for services provided to clients and understand CATN is a listing directory only with no liability for your professional conduct.
Deactivation request Submitting this form will send a request to remove your listing from the CATN directory. Your listing will be deactivated within 3–5 business days. This can be reversed — you can submit a new registration at any time to be relisted.
